2009-2010 Board of Governors (BOG) Fee Waiver Application (SPANISH)

• Use this form to apply for the BOG fee waiver.  This form is used for Summer 2010, Fall 2010, Winter 2011, and Spring 2011 terms at Los Angeles College.

 

2009-2010 Questionnaire/Educational Goal/Student Authorization Form

•  All students applying for financial aid must complete this form.

 

2009-2010 Verification Worksheet Form (INDEPENDENT)

2009-2010 Verification Worksheet Form (DEPENDENT)

• All students whose application has been selected for verification (an * showing by the EFC number located on the upper right hand corner of the Student Aid Report), must complete this form.

 

2009-2010 Academic Competitiveness Grant (ACG) Verification Form

• This form may be requested for recent high school graduates in order to verify eligibility for the Academic Competitiveness Grant (ACG).

  

2009-2010 Petition for Reinstatement of Financial Aid

• Use this form to petition for reinstatement of financial aid eligibility if you have been disqualified due to having a cumulative GPA below 2.0 and/or having completed less than 67% of your attempted units.

 

2009-2010 Petition for Extension of Financial Aid

• Use this form to petition for continued financial aid eligibility if you have been disqualified due to having attempted 90 or more units, having an Associate degree, and/or having a Bachelor degree or higher.
