Use this form to apply for the BOG fee waiver. This is used for Summer 2009, Fall 2009, Winter 2010, and Spring 2010 terms at Los Angeles Community Colleges.

All students applying for financial aid must complete this form.
All students whose application has been selected for verification (an * showing by the EFC number located on the upper right hand corner of the Student Aid Report), must complete this form.

Students whose Student Aid Report/Document Tracking Letter indicate that they may be eligible for the Academic Competitiveness Grant will need to download this form. The form will need to be completed by both the student and their high school Principal/Designee authority to verify eligibility.
Use this form to petition for reinstatement of financial aid eligibility if you have been disqualified due to having a cumulative GPA below 2.0 and/or having completed less than 67% of your attempted units.
Use this form to petition for continued financial aid eligibility if you have been disqualified due to having attempted 90 or more units, having an Associate degree, and/or having a Bachelor degree or higher.
