Library Instruction Request Forms

If you prefer, download the form and return your completed copy and return it to the library or fax it to us: (323) 241-5221.
You may also contact the library if you'd like to schedule a library instruction session for your class (323) 241-5235.
We will do everything possible to accommodate your Library Instruction requests, including coming to your classroom if you have internet access. Since we have fewer teaching Librarians now than in the past, we ask that you contact us as soon as you know when you might want to reserve a time slot.
Library Reserves Forms
There are several ways you can put a copy of your current textbook on reserve at the library:

We want to provide excellent service to our students and would appreciate your submitting your Library Reserve forms as soon as possible to avoid any delay in making these items available.
